Since its founding in 1998, C-Change has made the development and effective implementation of state, tribe/tribal organization, territory and Pacific island jurisdiction (PIJ) CCC plans a strategic priority. To further encourage the effective implementation of quality CCC plans and sustainment of CCC coalitions, in 2011 C-Change will present awards in each of the following categories:
(1.) CCC Coalition Impact Award will recognize one (1) state, one (1) tribe/tribal organization and one (1) PIJ CCC coalition that have successfully achieved evidenced-based and measurable impact in projects and/or interventions consistent with priorities in their respective CCC plan.
(2.) CCC Champion Award will recognize a volunteer CCC coalition member and/or partner that has gone above and beyond the call of duty to advance projects and/or interventions consistent with priorities in their respective CCC plan.
(3.) Exemplary CCC Leadership by an Elected Official Award will recognize an elected official that has been instrumental in advancing projects and/or interventions consistent with priorities in their respective state, tribe/tribal organization, territory or PIJ CCC plan or their active leadership and/or engagement in the CCC coalition.
JUDGING & SELECTION CRITERIA
A committee comprised of select organizational representatives from the Comprehensive Cancer Control National Partnership will review the nominations. Successful nominations will have thoroughly described how coalitions or individuals have met or exceeded the criteria below.
The CCC Awards Review Committee will select one recipient for each of the aforementioned awards utilizing a weighted scoring system based on a maximum of 100 points. Nominee(s) with the highest average score will be considered for an award.
Category I: CCC Coalition Impact Award – 100 points maximum
- Coalition demonstrates a positive outcome and/or systematic change that occurred as a result of implementing a specific CCC plan priority. [60 points]
- Coalition engaged and sustained member and partnership support and involvement in implementation of the specific CCC plan priority. [10 points]
- Coalition utilized evidence-based interventions in the process of implementing their CCC plan priority. [10 points]
- Implementation of the CCC plan priority was evaluated using a meaningful evaluation method. [10 points]
- Coalition’s ability to secure and maintain adequate monetary and non-monetary resources to sustain and enhance implementation of the CCC plan priority. [10 points]
Category II: CCC Champion Award- 100 points maximum
- Individual is actively involved in their CCC coalition. [10 points]
- Individual has a demonstrated commitment to CCC movement. [15 points]
- Individual’s actions have been instrumental in advancing the CCC coalition’s plan priorities [25 points]
- Individual goes or has gone above and beyond CCC coalition member requirements to advance CCC coalition’s priorities. [50 points]
Category III: Exemplary CCC Leadership by an Elected Official Award – 100 points maximum
- Elected official is actively involved with CCC coalition. [25 points]
- Elected official has been instrumental in advancing CCC coalition’s plan priorities through their elected position. [75 points]
ELIBILIBITY
The following are eligible to nominate a coalition and /or candidate for a CCC Award.
• CCC Coalition Chair
• CCC Program Director
• CCC Coalition Member
• C-Change member represented organization
• CCC National Partnership Organization
Only one (1) nomination can be received per person. Members of the CCC Awards Review Committee are not eligible to submit any nominations. Additionally, previous award recipients are not eligible to apply for an award in the category for which they have previously won for a period of three (3) consecutive years.
INSTRUCTIONS & DEADLINES
Before you begin, please thoroughly review the criteria for the specific award category you are nominating a coalition/ individual. All sections of the nomination form must be complete in order for the nomination to be considered. All nominations must be received by August 26, 2011. NOTIFICATION PROCESS
C-Change will notify award recipients via phone of their status by Friday, September 9, 2011. All other nominees will be notified of their status via mail by September 16, 2011.
In addition to being profiled on the C-Change website, award recipients will be honored at C-Change’s Annual Meeting, which will be held October 5-7, 2011 in at the Capital Hilton in Washington, DC. Upon request, coalition award recipients will have travel expenses reimbursed for up to two (2) coalition representatives to attend the meeting and accept the award on the coalition’s behalf. Also upon request, the Exemplary CCC Leadership by an Elected Official and Exemplary CCC Commitment by Coalition Member Award awardees will have their individual travel expenses reimbursed.
